Eleven-item self-report of upper-limb disability, transformed to a 0 to 100 score. Higher scores indicate greater disability.

Instructions

  • The patient rates each item for the past week, from 1 (no difficulty) to 5 (unable).
  • Transfer the patient's ratings from the completed form.
  • At most one item may be left unanswered. With two or more missing, no score can be calculated.
  • The optional work and sport/performing arts modules are not implemented in this version.
