Instructions
- The patient rates each item for the past week, from 1 (no difficulty) to 5 (unable).
- Transfer the patient's ratings from the completed form.
- At most one item may be left unanswered. With two or more missing, no score can be calculated.
- The optional work and sport/performing arts modules are not implemented in this version.
