Instructions

  • The patient rates difficulty with each activity from 0 (unable) to 4 (no difficulty), and each pain item from 0 (unbearable) to 4 (no pain).
  • Transfer the patient's ratings from the completed form.
  • The optional FADI Sport subscale is not implemented in this version.
